Managed from referral to outcome

Each referral follows a clear pathway designed around the child, the commissioned outcomes and the safeguards required for delivery.

Assess

We review the child's circumstances, requested provision, current education arrangement and relevant risks before accepting a referral.

Match

Practitioner allocation considers qualifications, curriculum needs, SEND/SEMH experience, location, availability and the individual child.

Mobilise

Required compliance, risk controls, safeguarding routes, timetable and commissioner authority are confirmed before provision begins.

Deliver

Individual provision is delivered against agreed learning and engagement objectives.

Monitor

Attendance, session evidence, progress and emerging concerns are captured throughout delivery.

Review & transition

Progress is reviewed with commissioners and provision adapts toward reintegration, transition or the agreed next destination.
