From referral enquiry to reintegration

A defined pathway, with the checks and approvals that must be satisfied at each stage before provision begins.

The pathway

Twelve stages, in order

  1. 1

    Referral enquiry

    A commissioner submits the initial provision request with the minimum information required to understand what is being asked for.

  2. 2

    Information review

    The case team reviews the request, the current education arrangement and the commissioned outcomes.

  3. 3

    Suitability, regulatory and risk assessment

    Requested hours, delivery environment, risk factors and regulatory boundaries are assessed before any acceptance decision.

  4. 4

    Practitioner search and matching

    Allocation considers qualification, specialism, SEND/SEMH experience, location and availability.

  5. 5

    Commissioner approval

    The proposed practitioner, timetable, outcomes and cost are confirmed with the commissioner.

  6. 6

    Initial programme and outcomes

    Learning and engagement objectives are agreed so progress can be evidenced from the start.

  7. 7

    Provision starts

    Delivery begins once compliance, safeguarding routes and commissioner authority are confirmed.

  8. 8

    Session information recorded

    Attendance, session content and engagement are captured for every session.

  9. 9

    Attendance or welfare concerns escalated

    Concerns are escalated according to the agreed protocol with the commissioner and, where required, statutory partners.

  10. 10

    Six-week review

    Attendance, progress and recommendations are reviewed with the commissioner.

  11. 11

    Programme continues, changes or transitions

    Provision is adjusted to reflect the child's progress and the commissioned outcomes.

  12. 12

    Reintegration, discharge or handover

    Provision closes with a structured handover to the receiving setting or next destination.

Submission of a referral does not equal acceptance. Ridgeway Provision confirms acceptance only after suitability, safeguarding, regulatory and practitioner availability considerations are satisfied.
